Supported Living

Elderly CoupleWe offer you a number of services to help you keep your independence and stay in your home.  We also provide accommodation that has been designed to make life a little easier and more secure for you.  If we cannot provide what you need to allow you to live independently we will tell you about other agencies that may be able to help. The following services are available to help you remain in your current property and keep your independence.

Disabled Adaptations

If you are an elderly person or have a disability, you may have difficulty doing certain things or moving around your home.  There may be alterations that can be carried out to your home to help you live your life as fully and independently as possible.  These permanent alterations are called ‘adaptations'. Adaptations can range from very simple things like grab rails and lever top taps to more specialist things like stair lifts or level access showers. If you are an elderly person or have a disability and you think you may benefit from having an adaptation made to your home, you should contact:

An Occupational Therapist or a Community Care Worker from Social Services will then arrange to visit you to assess your needs. If it is for a large adaptation (e.g. shower) a representative from HPCH will also    attend at this stage. Garden Maintenance Scheme

Gardens

We operate a free Garden Maintenance scheme to help certain people with their gardens.  If you have a condition which makes you physically unable to look after your garden yourself and you do not have any family or friends within the High Peak who can do your gardening for you, then you may qualify for assistance.  Contact your neighbourhood co-ordinator for a leaflet and an application form, call 0845 129 8075, click on 'Meet The Team' or email us at enquiries@hpch.co.uk

Carelink

Carelink Logo

Care link is a 24-hour helpline for older people and people with disabilities  who may need assistance day or night.  This service provides care and support at the press of a button.  If you ever have a problem and need help immediately, e.g. you become ill, or have a fall or an accident at home, you can get help by just pressing the button on your telephone unit or on your pendant.  A trained member of staff at our 24-hour call centre will answer your call promptly and they will get the help that you need.

Many of our flats and bungalows are already equipped with the Carelink System, but if you live in a house or would like the Carelink System installing, we can arrange to do this for a monthly charge.  We can also provide additional services known as Carelink Plus.  This includes help with shopping, preparing simple meals and household tasks.  Click here for more information or contact 0845 129 4877

Painting Partners

If you are physically unable to decorate your home, and you have no friends or family locally who can help, you may qualify to have a room decorated under the Painting Partners Scheme. Contact your neighbourhood co-ordinator for more details, call 0845 129 8075, click on 'Meet The Team' or email us at enquiries@hpch.co.uk

Accomodation

Eccles Fold

We have a number of flats and bungalows that are specifically for people over retirement age. All these properties have our emergency call system, Care Link, installed.  This means that residents can access help and support any time they need it.   We also provide sheltered accommodation, which is housing that has been designed with older people in mind. All our sheltered accommodation is secure, self-contained and offers help and support whenever needed via our 24-hour emergency alarm call service, Carelink.  Some sheltered housing schemes have communal lounges, kitchens and laundries.  Some schemes also have social clubs that arrange trips out and other social activities.
